Simplified new UAN based EPF Withdrawal Claim Forms (Form 19 UAN, Form 10C UAN, Form 31 UAN)
Note: Rcently, the EPFO has introduced Composite PF Claim Forms (Aadhar/ Non-Aadhar) applicable w.e.f. 20 Feb. 2017 and has withdrawn/ replaced Claim Forms 19, 10C and 31 (including UAN Forms 19, 10C and 31). Also, now partial PF withdrawals shall be allowed based on Self Certification (instead of other documents/ certificates):
The EPFO has notified that the employees whose details like AADHAAR Number and Bank Account Number have been seeded in their Universal Account Number (UAN) and whose UAN have been activated, may submit their PF withdrawal claims in Form 19 UAN, 10C UAN and 31 UAN directly to the regional EPF Office even without attestation of their respective employers. This new arrangement has been made by the EPFO with immediate effect.
Also, the EPFO has issued an Order containing instructions on implementation of the above initiative, which is based on Central Govt. directives, along with the formats of new / revised PF Withdrawal Claim Forms, as under:
Accordingly, the members/ employees under PF can use the new EPS/ EPF forms 10C UAN, 19 UAN and 31 UAN, for PF/ Pension withdrawal without attestation by respective employers. This new facility is available to members / employees under PF whose AADHAAR Number and Bank details have been seeded as KYC and have been duly verified by the employer using digital signature thereof and the details in form No. 11 (new) have already been completed / submitted. The Member / Employee has to attach a cancelled cheque containing the name of employee, bank account number, IFSC code of the bank / branch, etc. bank details with the above UAN based PF withdrawal claim forms to expedite the withdrawal process.
On the other hand, all other employees not fulfilling the above conditions shall continue to make their claims of withdrawal in existing form No. 19, 10C & 31, as the case may be.
It may be reiterated that EPS/ EPF withdrawal claim form 10C UAN, form 19 UAN and form 31 UAN need not be attested by employer and may be submitted by the employee directly to the respective jurisdictional EPF Offices.
Relaxation in the instructions for Mandatory use of UAN based PF Claim forms in certain cases by EPFO
The EPFO vide Circular dt. 13 July 2016 has made partial modification/ relaxation in the instructions for mandatory use of UAN based PF Claim forms in certain cases due to difficulties being faced by members, as under:

EPFO has clarified that the Declaration Form can be accepted without attestation by the employer both in the case of Claim Form 31 as well as UAN based Claim Form No. 31 (UAN), as under:
